Phoenix Support Services
Phoenix Support Services provides Individual Support Services for children and young adults with intellectual and/or developmental disabilities. Our knowledgeable staff are ready to provide hands-on skill building and assistance for your child within the comfort of your own home. Our experienced team of Supervisors and Direct Care Staff are passionate about providing personalized support and will work hard to help each child maximize their potential.

In addition to the skills of daily living and skill building, our Adolescent Program can provide assistance teaching skills that will help individuals live independently in community.

Who is eligible for these services?
This program was created and funded for children and young adults who have intellectual and/or developmental disabilities, are between the ages of 5 and 21, and have some level of difficulty functioning at home and/or in the community. This program is funded through the NJ Department of Children and Families and the children must be eligible for DDD/DCF/CSOC Services to receive services. You can check with your case manager or with PerformCare (877-652-7624) to determine your eligibility for the Individual Support Services (ISS) Program. You may choose to request Phoenix Behavioral Health’s Support Services if you would like to work with us.
